Map Issue

Hi,
I have installed this plugin and I have two issue:

Issue #1: The map on checkout page is not appeared in Chrome (http://prntscr.com/fq37tx) while its’ appeared on IE (http://prntscr.com/fq38ie).

Issue #2: The map not appeared in the admin order page on both Chrome & IE.
